REQUIREMENTS

  1. Filled up application form for BSU-JPLPC Admission test.

• For currently enrolled as Fourth year HS student- True Xerox copy of grades form 1st – 2nd grading period in Fourth Year certified by the HS Principal.

  1. Certified true copy of Form 138 (Report Card)

  2. Certificate of Good Moral Character (Xerox copy)

  3. Two (2) latest copies 2”x2” identical close-up photographs with applicant signature at the back.

  4. Non-refundable testing fee of P200.00

  5. Honorable Dismissal for Transferee

 
   

For Extension Trade Training Program

  1. Filled up application form for BSU-JPLPC Admission test.

  2. Certified true copy of Form 138 (Report Card) if the applicant is a high school graduate or certified true copy of transcript of records if applicant entered college.

  3. Two (2) latest copies 2”x2” identical close-up photographs with applicant signature at the back.

  4. Non-refundable testing fee of P200.00.

  5. Barangay clearance for out-of-school youth.

 
   

Filing of Application: Ongoing

Dates of Examination: Schedule will be given after submission of requirements.

 

What To Do

1.      Complete all requirements

2.    Pay your examination Fee of P200.00 at the Cashier’s Office and Present at the BSU-JPLPC Guidance Office.

3.    Secure and accomplish the application form for BSU Admission Test from the Guidance Office.

4.    Submit all the requirements and the accomplished application form for admission test to the Guidance Office.

5.    Ask your test permit.

6.    Take BSU Admission test.

7.    Result of the test shall be available one week after the examination at the Office of the Dean of the course you are enrolling. The Dean determines if you are qualified and gives you NOTICE OF PASSING. Submit this on enrollment together with other documents listed therein. You may also apply for reservation for your course because some courses may close if quota is reached.

8.    If you did not qualify for the course of your first choice, ask the Guidance Councilor for assistance regarding alternative choices available for you.

9.    Register/Enroll on dates specified.

 
   

Note: IF YOU LOSE YOUR TEST PERMIT, inquire at the BSU-JPLPC Guidance Office- at least one day before the test. Present proof that you had submitted your application and had paid your testing fee.

 
   

Filing of application for BSU-JPLPC Admission is at the Guidance Office during Office Hours 8:00 am. – 12:00 & 1:00 – 5:00 pm. From Monday to Friday excluding Holidays

Scholarship grants are available for deserving students. Please see the Scholarship Office.
